I am Authorized to request that the requested userid be enabled for COLA Userid Self Administration. This will allow me to Create new Userids, Activate and Inactivate Userids, Reset Passwords, and control if the respective userids may Place Orders, View Pricing, and see Purchase History summaries. While I understand that my Sales Representative will be double checking this request, I am here stating that I am either the owner of the business or have the owner's authorization to make this request.
